Guardian Life Insurance Company had a very dishonest business model. They sold life insurance policies to super-villains, with the full knowledge of who these criminals really were. Then when a super-villain policy holder died, the company refused to honor the policy and did not pay any money to the beneficiaries. They got away with doing this because the policy holder was a criminal, so the surviving family members couldn't go to the authorities or otherwise report the insurance company.

But when a minor super-villain known as the Tumbler died, and the company refused to pay benefits, the villain's mother died. The Tumbler's brother blamed the insurance company and investigated it. Eventually he as able to expose the company's dishonest business practices, with the help of Captain America.

Aside from the original Tumbler, other villains who died but the company refused to pay benefits for include: Dr. Octopus, Mr. Hyde, Cobra (Voorhees), and Jester (Powers). Note that some (all) of these individuals subsequently turned out to be alive (but the life insurance company had no way of knowing this..
